How to Back-Fill A Syringe

These small syringes are used to help dispense medication into the vagina where it is then absorbed. The syringe must first be filled with your prescribed medicated cream or gel before being placed in the vagina where the medication can be pushed out to the site of action. The syringe has easy to read numbers to ensure the right amount of medication is being used. Vaginal creams or gels are often utilized if a medication is unable to be delivered in a different dosage form.

How to Fill and Use a Backfilled Syringe

Step 1: Wash your hands thoroughly with warm soapy water

Step 2: Remove the plunger from the small syringe, and place the tip of the large syringe into the back of the small syringe (1).

Step 3: Press the plunger of the large syringe slowly.

Step 4: Continue until the small syringe is filled to the proper amount, use the plunger on the large syringe to determine the amount of cream you are putting into the smaller syringe.

Step 5: Return the plunger into the back of the small syringe.

Step 6: Remove your clothing to expose your vaginal area and get into a comfortable position.  Lie on your back with your legs slightly bent towards your belly this position will help expose the vaginal opening.  Alternatively, you can stand with knees slightly bent and feet a few inches apart. Carefully insert the tip of the applicator into the vagina.

Step 7: Slowly press the plunger until it stops, then removes the syringe.

Step 8: Wash your hands and the syringe (if reusing) thoroughly with warm soapy water.

Using your prescription should be painless and easy. Talk with your doctor or pharmacist if you have any questions or are having trouble using the cream properly. Discontinue use and check with your doctor or pharmacist if you notice any redness, itching, swelling, or other side effects not present before you started using this medicine.

Question: What should I do if I miss a dose?
Answer: If you miss a dose of your vaginal cream applies the missed dose as soon as possible. If your next dose is less than 4 hours away, then skip the missed dose and simply apply the next dose at the time that corresponds to the dosing regimen on your prescription label.

  1. Vaginal medications can be messy and many women may choose to wear a pad for a period of time following insertion.

  2. Try to avoid exercise or excessive movement for approximately one hour after insertion.
  3. Don’t use tampons when using a vaginal cream or gel. Tampons can absorb some of the medication preventing it from working accurately.
  4. Store medication in a cool, dry place out of the reach of children.
  5. How to use the syringe: If you have a 10 ml syringe and the prescription is written to apply 1 ml, then you would push the plunger of a full 10 ml syringe until it reaches the 9 ml mark – indicating 1 ml has been dispensed.
  6. Store this medication in a closed container, away from heat, moisture, and direct light and out of reach of small children and animals. 
  7. Precautions: If your prescription contains a hormone, the hormone can be transferred to another person if they rub or touch the skin where the medication was applied or if they touched your unwashed hand. You should wash the area well if you plan to have skin-to-skin contact with another person and dispose of your HRT products out of the reach of children and pets.  The locations of cream application are meant to be easily covered with clothes to avoid the transfer of cream to others. Virilization (changes in biological development) has also been reported in children with secondary exposure to some topical hormone preparations. Children should not come in contact with unwashed or unclothed application sites.
